  1. What’s the process for receiving Community Nursing support from True Ability?

    Fill out our referral form and select ‘Community Nursing’, and our team will be in touch to discuss your individual needs and explore options.

    What if I’m leaving hospital and require community nursing support?

    We can coordinate an in-person meeting at the hospital and create a tailored, Transitional Care plan, which will support you from your discharge from hospital to home.

    This ensures you can continue your recovery in a supported living arrangement and receive the right support, every step of the way. Fill out our referral form to get started. 

    What if I’m Already Receiving Supports from True Ability or other providers?

    You can still apply for Community Nursing, but we encourage you to discuss this with your Support Coordinator or a trusted family member to ensure you’re maximising the supports in your plan.

    What’s the Difference Between Community Nursing and High Care?

  2. Our Community Nursing team can support any routine medical needs, and even train support workers to deliver specific services. High Care provides regular, high-level support for more complex needs.

  3. My patient is eligible for NDIS and wants to use True Ability supports

  4. Having an NDIS Plan is a required pre-requisite to accessing True Ability supports and services, including Community Nursing.
    If your patient is on the NDIS already… please refer to the details in their NDIS plan and get in touch so that we can provide a health and wellbeing assessment to address your support requirements.

     

    If you believe your patient is eligible for NDIS... they will need an NDIS plan before supports can commence. Please work with the patient’s Health Liaison Offer (HLO) to begin the application process.
